Online new starter forms

Does anyone have experience or recommendations for creating online forms? Currently new starters are sent a Word document which is partly pre-filled with salary information, agreed working pattern, line manager etc. They then fill in their personal details and email it back to us. It is a bit of a clumsy system but it works. However in light of GDPR and a growing organisation we would like to start using something more technologically advanced and user-friendly. Any hints and tips gratefully received!
