Recruitment health questionnaire

Hi, do any of you use a recruitment health questionnaire for teaching/non-teaching staff? If so how do you determine which responses merit and occupational health review?

Does anyone use a professional occupational  health company to support this?

  • not teaching but when i worked in hr for a contact centre new starter health questionaires were given to our occhealth provider who decide who needed to get a review

    We have a very basic questionnaire that simply asks if any adjustments are required, which is then forwarded straight to our OH providers. If they consider that any adjustments may be required, we set up a meeting with the individual, their manager and OH to agree adjustments and any review periods. This is particularly important if staff need support in emergencies such as fire alarms.
  • We use a health questionnaire for our teaching and non-teaching staff. Any box ticked that indicates an issue means that we forward it to our adviser/GP for a review. He will then decide if it warrants a conversation or meeting with the individual and give us a steer about any adjustments needed.

    The doctor we use is just one that has a long association with the school - but we do have formal occupation health advisers that we have also used in the past for anything more than this kind of screening.
  • We use an online questionnaire. We send a link to access our occ health provider (Medigold) online portal. We don’t see the responses, just an outcome.
